Procedure Estimate

This estimate only approximates the cost of this visit for Puppy. It does not include any treatments that may be deemed necessary upon examination and commencement of the included treatments by your veterinarian. If such is discovered and deemed necessary for the health of Puppy, we will call for additional authorization. The following is a list of the treatments and/or supplies expected to be required during this visit and their approximate cost.

Understand, too, that your signature below indicates that you are the authorized owner and have reviewed and agree to the terms of this estimate. Your signature below does not make you responsible for the charges listed above unless performed upon Puppy. Payment is expected in full when picking up Puppy, we accept cash, Mastercard, Visa, Discover, American express and Care Credit. This estimate is good for 60 days.
